Key Responsibilities
  • Delivering nursing care to residents in post-acute care settings
  • Ensuring adherence to individualized care plans
  • Monitoring vital signs and reporting findings to supervisors as needed
  • Observing residents' conditions and communicating any concerns to the nursing team
  • Assisting with meal service and feeding as required
  • Supporting residents with daily living activities (ADLs) such as personal hygiene, mobility, and toileting
  • Maintaining cleanliness and hygiene in residents' living spaces
  • Facilitating communication between medical staff and residents, as well as with visitors
  • Assisting with the mobility of residents, including transfers and repositioning
  • Providing companionship and emotional support to residents
Qualifications and Skills
  • Current certification as a Patient Care Assistant
  • Basic knowledge of medical terminology and nursing procedures
  • Ability to lift at least 50 lbs
  • Strong organizational and communication skills
