Key Responsibilities- Delivering nursing care to residents in post-acute care settings
- Ensuring adherence to individualized care plans
- Monitoring vital signs and reporting findings to supervisors as needed
- Observing residents' conditions and communicating any concerns to the nursing team
- Assisting with meal service and feeding as required
- Supporting residents with daily living activities (ADLs) such as personal hygiene, mobility, and toileting
- Maintaining cleanliness and hygiene in residents' living spaces
- Facilitating communication between medical staff and residents, as well as with visitors
- Assisting with the mobility of residents, including transfers and repositioning
- Providing companionship and emotional support to residents
- Current certification as a Patient Care Assistant
- Basic knowledge of medical terminology and nursing procedures
- Ability to lift at least 50 lbs
- Strong organizational and communication skills
- Compassionate bedside manner
