We are seeking a compassionate and dedicated Hospice Aide/CNA to join our team at Halcyon Hospice, a part of LHC Group. As a Hospice Aide/CNA, you will play a vital role in providing personalized care and support to patients and their families.
Key Responsibilities:- Provide direct patient care services as established and delegated in the Plan of Care daily.
- Assist with activities of daily living (ADLs) such as mobility, transfers, walking, grooming, bathing, dressing, eating, or toileting.
- Perform incidental household services essential to the patient's health care at home.
- Participate in Quality Assessment and Performance Improvement (QAPI) plan or process.
- Successful completion of a competency evaluation prior to patient care.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
