Job SummaryThe successful candidate will be responsible for coordinating the Minimum Data Set (MDS) process, working closely with our interdisciplinary team to ensure accurate and timely completion of MDS assessments. This is a full-time position that requires a strong background in long-term care and rehabilitation nursing, with a focus on delivering exceptional patient care and outcomes.
Responsibilities- Coordinate the MDS process, ensuring accurate and timely completion of assessments
- Work closely with the interdisciplinary team to develop and implement individualized care plans
- Collaborate with nursing staff to ensure seamless delivery of care
- Participate in quality improvement initiatives to enhance patient outcomes
- Maintain accurate and up-to-date records of patient information
- NYS Licensed RN with a minimum of 5 years' experience in long-term care and rehabilitation nursing
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines
- Proficiency in SIGMACARE software and experience with MDS and PPS processes
- Ability to work independently and as part of a team, with a focus on delivering exceptional patient care and outcomes
