Job Title: Case Manager

Job Summary:

Hope Solutions is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will work closely with homeless individuals and families to provide support and guidance in achieving stable housing and self-sufficiency.

Key Responsibilities:

  • Conduct comprehensive assessments to identify client needs and develop individualized service plans.
  • Provide case management services, including housing search assistance, benefit navigation, and connection to community resources.
  • Collaborate with clients to set and achieve goals, and provide ongoing support and guidance.
  • Maintain accurate and up-to-date records of client interactions and progress.
  • Participate in team meetings and contribute to the development of program policies and procedures.

Requirements:

  • Bachelor's degree in social work or a related field.
  • Minimum of 2 years of experience in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and services.
