About the Role

We are seeking a skilled Case Manager to join our team at MercyOne Clive Rehabilitation Hospital. As a Case Manager, you will play a vital role in the rehabilitation process, working closely with patients, families, and healthcare teams to achieve optimal outcomes.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Conduct regular patient assessments and progress updates to inform treatment plans
  • Facilitate family conferences and caregiver communication to ensure patient and family engagement
  • Coordinate weekly patient care team conferences to refine treatment plans and achieve identified patient goals
  • Review patient records to identify potential comorbid conditions and communicate findings to the HIM team
Requirements
  • Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist)
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years experience in an inpatient setting, preferably in acute/rehabilitation hospital
  • Effective oral and written communication skills in English, with additional languages a plus
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
