Key Responsibilities:

  • Deliver professional nursing services to patients receiving hospice care
  • Evaluate the needs of patients and their families, providing appropriate support in line with medical directives
  • Work collaboratively with a multidisciplinary team and external organizations as needed
  • Maintain accurate patient documentation and update records as required
  • Engage in interdisciplinary group meetings and ensure consistent communication with the care team
  • Oversee the care plans of Certified Nursing Assistants
  • Provide continuous support to patients and families, especially during critical moments and bereavement
  • Participate in training and orientation initiatives
  • Contribute to the overall success of the organization through active collaboration

Essential Skills and Qualifications:

  • Proficient in assessing and addressing the needs of patients and families in diverse environments
  • Ability to maintain professional relationships with patients and their families
  • Effective communication skills, both verbal and written
  • Capable of fostering positive relationships with colleagues and the community

Required Qualifications:

  • Graduation from an accredited nursing program
  • Current licensure as a Registered Nurse in Georgia
  • A minimum of six months experience in acute care as an RN, with a preference for Home Care or Hospice experience
  • Valid CPR certification
  • Compliance with health clearance protocols and provision of professional references
