Hospice Care Nurse

Job Summary: We are seeking a compassionate and skilled Hospice Care Nurse to join our team at Affinis Hospice. As a Hospice Care Nurse, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.
Key Responsibilities:
- Assess patient needs and develop individualized care plans to meet their physical, emotional, and spiritual needs.
- Provide pain management and symptom control to ensure patient comfort and quality of life.
- Collaborate with the interdisciplinary team to coordinate care and services for patients and their families.
- Document patient progress and provide regular updates to the care team.
- Participate in ongoing education and training to stay current on best practices in hospice care.
Requirements:
- Current licensure as a Registered Nurse in the State of Georgia.
- At least six months of experience in acute care or hospice setting.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
