We are seeking a highly motivated and dedicated Community Care Clinician to join our team at NYC Health Hospitals/Community Care. As a vital member of our interdisciplinary team, you will deliver high-quality, patient-centered care to individuals in need, promoting health equity and social determinants of health.

">Job Responsibilities:">
  • Conduct thorough assessments of patient needs and develop individualized plans of care in collaboration with the healthcare team.
  • Provide evidence-based interventions, including education on disease management, medication adherence, and self-care techniques.
  • Collaborate with community resources and healthcare providers to ensure seamless care coordination and promote health outcomes.
  • Maintain accurate and comprehensive patient records, adhering to regulatory requirements and departmental standards.
">Requirements:">
  • A valid New York State license and current registration as a Registered Professional Nurse.
  • Two years of experience as a registered nurse, preferably in community health or a related field.
  • Basic Life Support (BLS) certification through the American Heart Association.
">Salary:">

$90,000 - $120,000 per annum, depending on experience.
