We are seeking a skilled Senior Community Care (SCC) Nurse Practitioner/Physician Assistant to join our team at Optum Home & Community Care. In this unique health care model, you will provide high-quality, cost-effective medical care to assigned members with both acute and chronic conditions.

About the Role:
  • Care Delivery: Provide exceptional patient care, conducting thorough assessments, documenting findings according to CMS regulations, and collaborating with healthcare teams to ensure optimal patient outcomes.
  • Comprehensive Assessments: Perform in-depth evaluations, leveraging your expertise to identify areas for improvement and develop targeted interventions.
  • Medication Management: Conduct initial medication reviews, annual medication reviews, and post-hospitalization medication reconciliations to minimize risks and enhance patient well-being.

Your Qualifications:
  • Nurse Practitioner Requirements: Certified NP through a national board; Master's degree in Nursing from an accredited program; ANCC or AANP certification as Adult-Gerontology Acute Care NP, Adult/Family, or Gerontology NP.
  • State Licensure: Active, unrestricted license in your state of residence.
  • Prescriptive Authority: Current DEA licensure or ability to obtain post-hire, per state regulations.
