Senior Care Management Coordinator

About the Role
This position plays a vital role in coordinating the care transitions of patients through assessment, planning, implementation, and evaluation of appropriate services and resources to maximize optimal outcomes.
As a Care Transition Specialist at Providence Health & Service, you will work collaboratively with multiple disciplines, both internal and external to the organization, to effect quality and cost-effective management of patient care along the continuum.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate with healthcare providers to ensure seamless transitions of care
- Identify and address potential barriers to care
- Monitor and evaluate patient outcomes
