Job Title: Hospice Aide (CNA)

At Liberty Hospice, we are seeking an experienced Hospice Aide (CNA) to join our team. As a Hospice Aide, you will provide direct patient care and support to our patients and their families.

Responsibilities:
  • Provide high-quality patient care and support to hospice patients in their homes.
  • Document patient care and progress accurately and thoroughly.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Provide education and support to patients and their families on end-of-life care and related topics.
Requirements:
  • High school diploma or equivalency.
  • Current listing with the Division of Facility Services Registry.
  • One year of patient care and record keeping experience in an acute care or custodial setting.
  • Documented proof of at least 75 certified education hours.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
