Gentiva Hospice is seeking a dedicated and compassionate Hospice Registered Nurse / RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes.
Responsibilities- Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and coordinating with healthcare teams.
- Act as a liaison between patients, families, and healthcare providers to ensure seamless care coordination.
- Empower patients and their families by educating them on care and support options.
- Collaborate with interdisciplinary teams to provide comprehensive care and support.
- Maintain accurate and detailed patient records.
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
