**Job Summary**

The Licensed Practical Nurse is a vital member of our interdisciplinary team, providing direct patient care under the supervision of a registered nurse. This role requires a strong commitment to delivering high-quality, patient-centered care in a compassionate and respectful manner.

**Key Responsibilities**

1. Provide direct patient care as defined in the State's Nurse Practice Act, ensuring the highest level of care and safety for our patients.
2. Implement the plan of care initiated by the registered nurse, working collaboratively with the healthcare team to achieve positive patient outcomes.
3. Document patient information accurately and timely, consistent with the plan of care, to ensure seamless communication among team members.
4. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care, empowering patients to take an active role in their healthcare.
5. Participate in the coordination of home health services, identifying and reporting the needs of other disciplines to the registered nurse and/or Clinical Supervisor.
6. Utilize equipment and supplies effectively and efficiently, minimizing waste and optimizing patient care.
7. Engage in ongoing personal and professional growth and development, staying current with best practices and industry developments.
8. Perform other duties as assigned by the registered nurse, demonstrating flexibility and adaptability in a dynamic healthcare environment.

**Requirements**

1. Graduate of an accredited practical nurse or vocational nursing program, with a strong foundation in nursing principles and practices.
2. Three years of nursing experience, with a preference for community/home health or medical/surgical experience.
3. Current licensure as a licensed practical nurse or licensed vocational nurse in the state, with a commitment to upholding professional standards and practices.
