Key Responsibilities:

  • Conduct comprehensive clinical assessments of patients to determine their eligibility for the appropriate level of care and services.
  • Collaborate with healthcare professionals, patients, and their families to gather insights and provide holistic care.
  • Develop informed Level of Care recommendations based on thorough clinical evaluations, ensuring compliance with contractual and regulatory standards.
  • Utilize advanced technology to efficiently manage and track assessment activities.

Requirements:

  • Active, unrestricted Registered Nurse (RN) License in the state of New Hampshire.
  • Advanced clinical assessment and critical thinking abilities.
  • Exceptional verbal and written communication skills.
  • Proven track record of working effectively within a team-oriented environment.
  • Proficiency in Microsoft Excel, Word, and Outlook.
