Nurse Practitioner

The Nurse Practitioner collaborates with Physicians to provide high-quality primary health care services, differentiating between normal medical findings and those requiring treatment consultation or referral. This role involves evaluating and monitoring health status, performing diagnostic and therapeutic services, and providing age-appropriate education and counseling for disease prevention, disease management, and health maintenance.
Responsibilities- Evaluate and monitor health status, performing diagnostic and therapeutic services as needed
- Provide age-appropriate education and counseling for disease prevention, disease management, and health maintenance
- Collaborate with Physicians to coordinate a continuum of care for patients
- Prescribe medications within the scope of practice and coordinate referrals for specialty care or social service support
- Registered Nurse with additional training through an advanced studies program
- Experience in ambulatory care/primary care setting
- Licensed to practice within the State of Wisconsin and certified through a national professional nursing organization
- Family practice experience
- Knowledge of current practice standards for Nurse Practitioners
- Ability to provide age-appropriate care to patients of all ages
- Ability to collaboratively work with patients' Physicians in coordinating a continuum of care
- Ability to establish and maintain harmonious and cooperative working relationships with other people
This role is based in a normal clinical practice setting and may involve exposure to risk of blood-borne diseases, electrical or mechanical hazards from office medical equipment, and unpleasant patient elements.
