Job Summary:

We are looking for a dedicated and experienced RN to conduct in-home nursing visits, provide case management, and oversee a team of RNs, LPNs, and caregivers. If you are passionate about delivering exceptional patient care and have a strong background in home health nursing, we encourage you to apply.

Responsibilities:
  • Conduct in-home nursing visits for clients, including physical assessments, medication management, and home safety evaluations.
  • Provide case management services to assigned clients, including developing care plans and coordinating with healthcare providers.
  • Oversee a team of RNs, LPNs, and caregivers to ensure high-quality care is delivered to clients.
  • Collaborate with regional directors and other personnel to identify and resolve issues, improve services, and ensure compliance with agency policies.
  • Participate in interviewing, selecting, and evaluating clinical personnel, as well as providing training and education to staff.
  • Review and evaluate clinical documentation for accuracy and completeness, and participate in performance improvement initiatives.
Requirements:
  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years of nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.
