Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at The Salvation Army Central Territory. As a Case Manager, you will play a vital role in coordinating social services activities, providing direct assistance and referrals to clients in need, and assessing families for eligibility in our Pathway of Hope program.

Key Responsibilities
  • Conduct outreach and engagement activities to identify eligibility requirements and build rapport with target populations.
  • Perform intake assessments, develop case plans, and provide regular meetings with participants to review goals and objectives.
  • Provide information and referral services, assist participants in connecting to community resources, and offer advocacy services as needed.
  • Collaborate with staff and contracted services to meet individual and family needs, and develop discharge plans to ensure permanent housing stability and economic stability.
  • Maintain comprehensive case files, complete required documentation, and ensure client confidentiality in accordance with established procedures and regulations.
Requirements
  • Bachelor's degree in a human service area or extensive experience in strength-based case management.
  • Minimum of two years of case management experience in a comparable social service program.
  • Strong communication and interpersonal skills, with the ability to work with diverse populations and build relationships with community agencies.
  • Ability to think logically and critically, solve complex problems, and make appropriate judgments and decisions.
  • Fluency in English, with the ability to speak and understand Spanish preferred.
