Care Coordination Role

The RN Care Coordinator plays a vital role in ensuring seamless transitions for patients, promoting quality care, and optimizing outcomes. This position requires strong clinical expertise, effective communication, and collaboration with various stakeholders to progress care towards optimal results.

Key Responsibilities
  • Conduct discharge planning assessments and develop multidisciplinary plans to meet patients' clinical and psychosocial needs.
  • Collaborate with healthcare teams and post-acute service providers to ensure timely and smooth transitions to appropriate care settings.
  • Identify and mitigate risks for readmission, implementing interventions to promote patient safety and well-being.
  • Deliver patient notifications and related documentation, while providing education and advocacy to support patients' needs.
  • Participate in performance improvement initiatives and adhere to hospital policies, standards of practice, and regulatory requirements.
Qualifications
  • Graduate of an accredited nursing program.
  • Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing.
  • RN license in the state(s) covered.
  • BLS certification within three months of hiring.
Preferred Qualifications
  • Bachelor's Degree in Nursing or related healthcare field.
  • At least five years of nursing experience.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification.

Pay Range: $47.43/hour
