Your Responsibilities:



  • Develop and implement comprehensive care plans for patients and their families
  • Collaborate with community resources to plan services for individuals and families
  • Conduct initial, comprehensive, and ongoing assessments of patient and family needs
  • Document accurate clinical notes regarding patient condition and care given
  • Travel throughout the day and sometimes after-hours

Requirements:



  • RN licensure for the state(s) you work in
  • Clean driving record and valid driver's license and auto insurance
