">Job Title: Compassionate Hospice Care Specialist
">Estimated Salary: $65,000 - $80,000 per year
">Job Summary:
">We are seeking a compassionate and skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will be responsible for providing emotional, social, and spiritual support to our patients and their families, helping them navigate the challenges of terminal illness and end-of-life care.
">Main Responsibilities:
">- ">
- Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.">
- Communicate findings to the registered nurse and other members of the interdisciplinary group.">
- Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse, and plan intervention based on evaluation findings.">
- Counsel patients and families/caregivers as needed in relationship to stress and other identified coping difficulties.">
- Provide crisis intervention when necessary.">
- Assess for, and educate the interdisciplinary group, on any special needs related to the culture of the patient and family.">
- Work under the supervision of a Masters Prepared Social Worker (MSW).">
- Maintain clinical records on all patients referred to social work.">
- Assist patients and families in preparation of advanced directives.">
- Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.">
- Provide information to patients and families/caregivers and community agencies.">
- Serve as liaison between patients and families/caregivers and community agencies.">
- Maintain collaborative relationships with organization personnel to support patient care.">
- Maintain and develop contracts with public and private agencies as resources for patient and personnel.">
- Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings.">
- Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.">
- Will assist the Director of Bereavement in maintaining a bereavement program to meet the needs of families/caregivers for 13 months following the death of a patient.">
- Ensure the initial bereavement assessment is completed, or assigned to appropriate team member.">
- Assign and oversee volunteers making bereavement calls.">
- Assist with maintaining, organizing, and updating bereavement support group schedule.">
- Actively participate in quality assessment performance improvement teams and activities.">
- Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.">
- Other duties as delegated by the Clinical Supervisor.">

Requirements:
">To be successful in this role, you will need:

- Bachelor's degree in social work from an institution accredited by the Council on Social Work Education; Master's degree in social work is preferred.">
- License in state(s) of practice preferred.">
- One (1) year experience in healthcare preferred.">
- Ability to pass DHS background study.">
- Valid State Driver's License, automobile in good working order, automobile insurance in accordance with state and organization requirements, and an acceptable driving record required.">
