Job Summary: We are seeking a highly skilled Care Transition Coordinator to join our Case Management team at Roswell Park Comprehensive Cancer Center. The successful candidate will support critical and time-sensitive functions, including managing post-acute care referrals and communicating with patients, families, and stakeholders. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Manage post-acute care referrals and ensure timely scheduling of clinical needs
  • Communicate directly with patients, families, and stakeholders to ensure seamless care transitions
  • Proactively identify and address potential barriers to care
  • Collaborate with the Case Management team to develop and implement effective care plans

Requirements:

  • Associate's degree in a business or health-related field
  • 2+ years of experience in healthcare or a related field
  • Strong organizational and communication skills
  • Ability to work effectively in a fast-paced environment
