As a RN Case Manager PRN, you will play a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. You will provide, coordinate, and direct care specific to the needs of each Rehab patient, collaborating with the Rehab Program Director and Facility Case Management Director regarding departmental functions.

Key Responsibilities:

  • Develop and implement patient-centered treatment plans that support individual strengths, abilities, needs, and preferences.
  • Facilitate patient involvement throughout the rehabilitation process, ensuring their needs and preferences are met.
  • Document findings from Discharge Planning Evaluations and psychosocial assessments, communicating social, financial, or discharge needs and preferences to relevant parties.

Requirements:

  • Associate's Degree in Nursing required.
  • One year of Rehab or Med/Surg experience preferred.
  • Experience in Utilization Review, Discharge planning, or Case Management preferred.
