Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at At Home Healthcare. As a Home Health LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Key Responsibilities
  • Provide Quality Patient Care: Deliver high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Documentation and Care Delivery: Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs, visit assignments, and schedule changes.
  • Quality and Professionalism: Actively participate in the performance improvement plan, work to reduce patient hospitalizations, and improve outcomes. Attend and participate in all meetings, including case conferences, and maintain a positive attitude.
  • Teamwork and Adherence: Adhere to and participate in the agency's Episode Management process and perform all other duties as assigned.
Requirements
  • Current LPN Licensure: Current licensure in the state of practice.
  • Driver's License and Vehicle: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification: CPR certification required.
