At Liberty Home Care, we are seeking an experienced Occupational Therapist to join our team. As a Home Care Occupational Therapist, you will provide patient care services, including direct treatment and follow-up, to patients in their homes.

Key Responsibilities:

  • Evaluate patients and develop a plan of care based on their diagnosis and assessment findings.
  • Provide patient care services, including direct treatment and follow-up, to patients in their homes.
  • Coordinate with other healthcare professionals, including COTAs and Patient Care Coordinators, to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare teams to ensure understanding and compliance with treatment plans.
  • Document patient care data accurately and thoroughly, reflecting evidence of improvement, progress, or decline in patient condition.

Requirements:

  • Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy.
  • Successful completion of the National Certification Exam and current licensure from the state where therapy will be provided.
  • Prefer a minimum of one year's therapeutic experience in a non-home health care environment.
  • Strong knowledge of theory and concepts relating to the practice of occupational therapy.
  • CPR certified and strong communication and organization skills.
