Job DescriptionWe are seeking a full-time Nurse Care Coordinator (LPN) to join our team in Voorhees, NJ. As a key member of our care team, you will be responsible for ensuring Ennoble Care provides high-quality care services. You will work closely with clinicians, staff, and patients to achieve healthcare goals and maintain open communication. This role requires a compassionate, experienced, and highly organized individual who can manage individual care providers, including scheduling and providing support for caregivers and families.
Responsibilities:- Develop and implement individualized patient care plans using Ennoble Care's electronic medical record system.
- Provide regular communication with patients to coordinate care, offer support, and ensure compliance with care management programs.
- Document all client interactions accurately and maintain comprehensive care plans.
- Collaborate with Primary Care Clinicians and other team members to address patient needs and ensure accurate care plans.
- Keep Team Supervisors informed of all issues related to care plans and known or perceived issues.
- Demonstrate ability to work with various cross-organizational areas to meet patient needs.
- Develop skills in using technology, including secure email, telephone systems, and electronic medical records.
- Adhere to documentation protocols and best practices for daily work logs, client issues, and internal communications.
- Provide excellent customer service, as demonstrated by positive feedback from customers and patients.
- Contribute to a positive team environment by supporting all members in a productive and constructive manner.
- Utilize a computer, telephone, copy machine, and other office equipment as necessary.
- Must be a Licensed Practical Nurse (LPN).
- Must be comfortable with phone communication for extended periods.
- Must be compassionate and empathetic towards patients, demonstrating exceptional customer service.
- Ability to take accurate notes to document tasks in a timely manner.
- Ability to multitask between patients and workstreams while remaining organized and efficient.
- Ability to thrive in a fast-paced environment.
- Must be able to work from 8:00 - 4:30pm, Monday through Friday.
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs.
- Knowledge of basic healthcare terms, conditions, roles, and care principles is helpful.
- Candidate must be able to pass a drug screen, background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis.
