Nurse Care Coordinator

Job Summary:
Virtua Health is seeking a skilled Registered Nurse to join our team. As a key member of our healthcare team, you will be responsible for assessing, diagnosing, planning, implementing, and evaluating patient care through outcome identification and evidence-based practice.
Key Responsibilities:
- Perform an assessment of the patient, leading to development, implementation, and evaluation of a plan of care.
- Administer, monitor, and document therapeutic interventions and regimes.
- Effectively manage rapidly changing situations.
- Participate in clinical decision-making.
- Educate patients and families regarding disease processes, individual care needs, wellness, safety issues, etc.
- Delegate and monitor care rendered by other members of the nursing department and healthcare team.
- Demonstrate standard precautions and patient safety principles in practice.
- Participate in orientation, education, and development of healthcare team members.
- Identify and participate in performance improvement activities.
- Maintain professional and departmental level competencies.
Requirements:
- Must have a valid NJ RN license.
- Demonstrate knowledge of nursing skills, hospital practices, procedures, and standards.
- Perfect skills and stay abreast of current nursing practice through ongoing education, journals, etc.
- Must have strong communication skills.
- Must be able to coordinate efforts of a team of care providers.
