Veterans Support Specialist

About the Role
This position offers an exciting opportunity to work with Tempus Unlimited Inc. as a Veterans Case Manager. The successful candidate will be responsible for providing comprehensive care management services to military veterans who require assistance with daily living tasks and transitioning to community living.
As a member of our team, you will play a vital role in ensuring that our veterans receive the necessary support and resources to thrive in their communities. Your exceptional organizational skills, attention to detail, and ability to maintain confidentiality will be essential in this role.
Key Responsibilities
- Care Management Services: Conduct referrals, intake, and orientation, as well as ongoing case management services to support our veterans' transition to community living.
- Regulatory Compliance: Ensure compliance with all relevant regulations and guidelines, including VDC rules and regulations.
- Documentation and Reporting: Provide accurate and timely documentation, including monthly reports to VDC VA Case Managers.
- Team Collaboration: Work closely with our team to ensure seamless coordination of services and optimal outcomes for our veterans.
Requirements
- Military Experience: Previous experience working with military personnel or veterans is highly desirable.
- Community Services Background: A background in providing community services to individuals with disabilities is essential.
- Budget Planning: Ability to manage budgets and prioritize spending to meet the needs of our veterans.
- Communication Skills: Excellent written and verbal communication skills, with the ability to build strong relationships with veterans, family members, and healthcare professionals.
Working Environment
This role requires regular travel to various locations, including the homes of our veterans, facilities, and community settings. A valid driver's license and reliable transportation are essential for this position.
