Home Infusion Pharmacist Job Summary

As a Home Infusion Pharmacist, you'll play a critical role in delivering care and improving health outcomes for our patients. We're seeking a highly skilled and compassionate pharmacist to join our team and contribute to our mission.

Key Responsibilities:
  • Dispense unit doses, specialty pharmaceuticals, injectables, IV, enteral, and related supplies, adhering to state and federal regulations and company policies.
  • Compound medications using appropriate components, supplies, and sterile/ventilated environments.
  • Process, pack, and ship outgoing pharmacy orders, ensuring drug, related supplies, and patient collateral are included.
  • Perform drug/supply ordering and inventory/warehouse duties, including proper storage of drugs and supplies and appropriate refrigeration of perishable products.
  • Review medication orders and evaluate their appropriateness and completeness, considering the special needs and behaviors of the specific age group served.
  • Verify all prescriptions not directly received from the prescriber or physician's agent.
  • Provide oversight of the safety, appropriateness, and effectiveness of pharmaceutical care provided to patients as it relates to specialty products dispensed.
  • Assess patients at the initiation of drug therapy, develop patient care plans, and monitor patients according to the care plan throughout the course of therapy.
  • Communicate with patients, physicians, nursing staff, other pharmacists, and other members of the healthcare team regarding patient status, response to therapy, and clinical objectives to ensure clinical outcomes are met.
  • Compound intravenous and specialty pharmaceuticals in an aseptic fashion according to established State Board of Pharmacy, Federal, USP sterile guidelines, and company policy.
  • Knowledgeable of the proper use and handling of both sterile and hazardous medications; compound and handle sterile and hazardous medications in accordance with established guidelines and regulations.
  • Ensure compliance with all FDA, DEA, State Board of Pharmacy, OSHA, and other regulatory standards; follow all department policies and procedures.
  • Provide indirect oversight of pharmacy technician and pharmacy support staff.
  • Pharmacists monitor and verify all prescription orders upon creation and dispensing. Calibrate dosing pumps as applicable.
  • Interpret provider prescriptions and create orders, access, input, and retrieve prescription and medical/lab information from the computer system, verify whether orders/drugs are reimbursable/covered.
  • Contact members/patients to validate data, resolve rejected claims, request payment information, establish priority of order or delivery details, consult on medication issues, and set up treatment in on-site facilities.
  • Prep and scan documents, verify member/patient data, submit prescription orders in the computer system.
Requirements:
  • BS in Pharmacy or PharmD degree.
  • Current, unrestricted Pharmacist license in the state of Washington.
  • Solid drug product and clinical pharmacy knowledge.
  • Knowledgeable in various infusion-related therapies with the ability to work through the complexity of therapy-related orders.
  • Proficient with Microsoft Office.
  • Ability to fill medication orders accurately and in a timely manner.
  • Ability to work a flexible schedule (8-hour shifts between hours of 7am-7pm weekdays with rotating weekends) and rotate being on call based on business needs.
  • Permanent residence within a commutable distance to the branch.
Preferred Qualifications:
  • Nutritionally Certified.
  • Pharmacist with Home Care or Home Infusion experience.
  • Sterile compounding experience.
  • CPR+ software experience.
Physical Requirements:
  • Sitting, standing, walking in an office/pharmacy operation environment.
