We are seeking a skilled and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in the comfort of their own homes.
Key Responsibilities- Provide direct patient care under the supervision of a Registered Nurse, following a personalized care plan.
- Assist patients with daily living activities, such as bathing, grooming, and dressing.
- Help patients with medication management, including opening bottle caps and reading labels.
- Take vital signs, such as temperature, pulse, and blood pressure, and report any abnormalities to the supervisor.
- Assist with feeding patients, including preparing meals and feeding or assisting with self-feeding.
- Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation.
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Must meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation.
- At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
