Job Title: Contract Liaison

At PACE Southeast Michigan, we are seeking a highly skilled and experienced Contract Liaison to join our team. As a key member of our Interdisciplinary Team, you will play a vital role in establishing and maintaining collaborative working relationships with our external stakeholders, including nursing homes, assisted living facilities, group homes, dialysis centers, and contracted providers.

Key Responsibilities:
  • Conduct monthly walk-throughs at assigned facilities to evaluate participant care and identify areas for improvement.
  • Provide education and training to contracted providers on the PACE model, policy, and procedures.
  • Establish and maintain good working relationships with internal and external staff to ensure effective communication and collaboration.
  • Coordinate communication between PACE SEMI and contracted facilities to ensure seamless care transitions.
  • Perform annual oversight of contracted facilities, including competency assessments, basic life support training, CLIA verification, and facility oversight reviews.
  • Serve as the primary point of contact for all contracted facility-related matters and communicate with PACE SEMI IDT members as needed.
Requirements:
  • Current Michigan licensure as a Licensed Practical Nurse (LPN).
  • Experience working in a nursing home setting.
  • Knowledge of Utilization Review and Discharge Planning processes.
  • Ability to work with diverse populations and maintain accurate records.
  • Excellent written and verbal communication skills.
