Position Overview:


The Clinical Pharmacist is responsible for delivering comprehensive pharmaceutical care to outpatient populations, adhering to established professional standards and regulatory requirements. This role involves overseeing the operational activities of the pharmacy within a closed system, guiding pharmacy technicians and interns in their duties. The pharmacist ensures precise medication preparation and distribution, all while managing time-sensitive tasks effectively. The position also entails developing safe, rational, and cost-efficient medication management plans aimed at enhancing patient outcomes. The pharmacist will monitor drug therapy and utilization throughout the care continuum and evaluate medication therapy results. Additionally, collaboration with patient assistance programs is essential to provide medications to underserved populations. Patient and provider education is a key component of this role, along with participation in initiatives to elevate public health awareness.
KEY RESPONSIBILITIES

  • Interprets and assesses all prescriber medication orders to confirm suitable drug therapy for each patient, in compliance with legal and regulatory standards.
  • Reviews medication orders for therapy appropriateness, including dosage, form, administration route, schedule, initiation time, and duration.
  • Monitors patient medication regimens for potential allergies, drug interactions, contraindications, and adverse drug reactions.
  • Conducts patient medication histories and counseling, providing educational materials as necessary.
  • Consults with healthcare professionals to develop safe, rational, and cost-effective medication management plans.
  • Ensures compliance with all pharmacy practice regulations as defined by relevant governing bodies.
  • Supervises pharmacy technicians, interns, aides, volunteers, and students in medication preparation, packaging, labeling, and distribution.
  • Compounds and dispenses medications and injectables, recognizing the critical nature of accuracy in these processes.
  • Generates and reviews dispensing reports and accounting documentation.
  • Initiates and leads specific departmental projects as needed.
  • Adheres to and enforces the Formulary Medication System and departmental policies.
  • Maintains effective working relationships and approaches conflicts constructively, contributing to problem identification and resolution.
  • Upholds the confidentiality of patient, physician, employee, and visitor information, discussing sensitive information only with authorized personnel in private settings.
  • Acts in alignment with the Mission, Vision, and Values of the organization.
  • Takes responsibility for performing job duties safely, ensuring personal and coworker safety, and promptly reporting hazards.
  • Generates quality and accounting reports from the pharmacy management system.

ADDITIONAL RESPONSIBILITIES

EDUCATION, EXPERIENCE, AND LICENSURE REQUIREMENTS

Education:
A Bachelor’s degree in Pharmacy is required. A Pharm.D. or other advanced degree is preferred.

Experience:
Prior experience in community or retail pharmacy settings is preferred.

Licensure:
A current Pharmacist license in the State of Michigan or eligibility for licensure is required.

SKILLS AND ABILITIES

  • Comprehensive knowledge of medications, compounding, and dispensing practices. Strong understanding of pathophysiology, pharmacokinetics, and therapeutics.
  • Interpersonal skills essential for effective interaction with patients, physicians, and healthcare providers.
  • Analytical skills necessary for organizing and prioritizing multiple responsibilities.
  • Basic computer skills, including operations and data retrieval.
  • Effective communication skills for clear written and verbal interactions.
  • Thorough understanding of the regulatory, statutory, and professional aspects of pharmacy practice.
