Position Overview:
The Clinical Pharmacist is responsible for delivering comprehensive pharmaceutical care to outpatient populations in alignment with professional standards and regulatory requirements. This role involves overseeing the operational functions of the pharmacy, including the management of pharmacy technicians and interns. The pharmacist ensures precise medication preparation and distribution while adhering to strict timelines. The position also entails developing safe, effective, and economical medication care plans aimed at enhancing patient outcomes. The pharmacist will manage drug therapy and utilization throughout the care continuum and assess medication therapy results. Additionally, the role includes collaborating with personal assistant programs to obtain medications for economically disadvantaged patients and providing education to both patients and healthcare providers. Engaging in professional activities to promote public health awareness is also a key responsibility.
KEY RESPONSIBILITIES
- Interprets and assesses all prescriber medication orders to guarantee suitable drug therapy for each patient, complying with legal and regulatory standards.
- Evaluates medication orders for appropriateness of therapy, including dosage, form, administration route, schedule, initiation time, and duration.
- Monitors patient medication regimens for potential allergies, drug interactions, contraindications, and adverse drug reactions.
- Conducts patient medication histories and counseling, providing educational materials as necessary.
- Consults with and advises healthcare professionals in formulating safe, effective, and economical patient medication care plans.
- Adheres to all pharmacy practice regulations as mandated by state and federal laws.
- Supervises pharmacy technicians, interns, aides, volunteers, and students in the preparation, packaging, labeling, and distribution of medications.
- Compounds and dispenses medications and injectables, recognizing the critical nature of accuracy in these processes.
- Generates and reviews dispensing reports and financial records.
- Initiates and coordinates specific departmental projects as needed.
- Upholds and enforces the Formulary Medication System and departmental policies.
- Maintains positive relationships and effectively addresses conflicts, contributing to problem-solving efforts.
- Ensures confidentiality of all patient, physician, employee, and visitor information.
- Exemplifies the Mission, Vision, and Values of the Health System in all professional interactions.
- Takes responsibility for performing job duties safely, reporting hazards and unsafe practices to management.
- Generates quality and financial reports from the pharmacy management system.
ADDITIONAL RESPONSIBILITIES
Performs other duties as assigned.
EDUCATION, EXPERIENCE, AND LICENSURE REQUIREMENTS
Education:
A Bachelor’s degree in Pharmacy is required; a Pharm.D. or related advanced degree is preferred.
Experience:
Previous experience in community or retail pharmacy is preferred.
Licensure:
A current State of Michigan Pharmacist license or eligibility for licensure is required.
SKILLS AND COMPETENCIES
- Comprehensive knowledge of medications, compounding, and dispensing practices, along with a solid understanding of pathophysiology, pharmacokinetics, and therapeutics.
- Strong interpersonal skills for effective interaction with patients, physicians, and healthcare providers.
- Analytical skills to prioritize and manage multiple responsibilities efficiently.
- Basic computer proficiency, including data retrieval and operations.
- Effective communication skills for clear written and verbal exchanges.
- Thorough understanding of the regulatory and professional standards governing pharmacy practice.
