Palliative Care Nurse Practitioner Opportunity

NYC Health + Hospitals/Jacobi is a leading healthcare institution seeking an experienced Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will provide high-quality patient care to individuals with serious illnesses, focusing on relief from pain and symptoms, and improving their quality of life.

Responsibilities:
  • Provide direct and indirect patient care to palliative care outpatients and inpatients in critical care and general med/surg units.
  • Develop and implement individualized care plans to address patients' physical, emotional, and spiritual needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Conduct thorough assessments, develop treatment plans, and provide education to patients and families.
  • Manage pain and non-pain symptoms, and provide support to patients and families.
Requirements:
  • A valid New York State license and current registration to practice as a Registered Professional Nurse.
  • A current certificate as a Nurse Practitioner issued by the New York State Department of Education (NYSED).
  • A current certification issued by the State Education Department to prescribe medication.
  • Master's degree or equivalent in education and experience, plus one to two years of hospice palliative care experience.
  • ACHPN certification (or eligible in 1 year).
  • Excellent communication skills.
  • Demonstrate competence in pain and non-pain symptom management.
