Job Summary

We are seeking a skilled RN to join our team as a Phone Triage RN in the Heart Center department. As a Phone Triage RN, you will be responsible for triaging patients over the phone or in the office to ensure prompt identification of patients with high-risk conditions.

Key Responsibilities
  • Talk directly to patients on the telephone, providing a physician with necessary information and directing them to emergency rooms or urgent care centers or to see their physician during office hours as instructed by a physician.
  • Perform short evaluations of patient situations to estimate the severity of illness and/or injury, including learning about chief complaints and, as possible, obtaining vital sign and mental status information.
  • Confer with physicians to determine the best course of action for patients.
  • Send patients with high-risk chief complaints, such as chest pain, to the ER immediately as directed by physicians.
  • Set up appointments for patients who do not need to go to the ER but need to see a physician or arrange for an appointment scheduler to make the appointment.
  • Process prescription renewals as directed by physician orders.
  • Liaise between referring physicians calling the office to speak with cardiologists regarding mutual patients.
  • Liaise between hospitals and cardiologists, disseminating requests for consults or transfers by rounding physicians and entering these patients into the portal for rounding cardiologists to see.
  • Participate on the Emergency Response Team.
  • Perform IV Lasix administration.
  • Maintain and model REACH values (Respect, Excellence, Accountability, Compassion, Honor).
  • Demonstrate regular, reliable, and predictable attendance.
Requirements
  • Associates degree in nursing or bachelor's degree.
  • Minimum one-two years' experience in nursing triage or equivalent experience (cardiology/ICU/ED/OR experience preferred).
  • Knowledge of telephone-based clinical assessment techniques.
  • Knowledge of medical practice telephone triage protocols.
  • Knowledge of appropriate home health information for patients to follow until visit with physician, if they do not need an immediate ER visit.
  • Skill in using electronic medical records to check patient history.
  • Skill in using computerized medical information database during evaluation as a guide to appropriate decision.
  • Skill in making triage decisions and responding quickly and calmly in emergency situations, such as calling 911 and arranging for an ambulance.
