Job Summary

We are seeking a highly skilled and compassionate Chronic Condition Management Specialist to join our team. As a key member of our healthcare team, you will be responsible for developing and implementing personalized care plans for patients with chronic medical conditions.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' medical and psychosocial status, identifying areas for improvement and developing tailored care plans to meet their unique needs.
  • Coordination and Implementation: Collaborate with healthcare providers, patients, and families to coordinate care, ensure timely and efficient medical treatment, and promote patient self-management of chronic conditions.
  • Education and Support: Provide patients and their families with education, guidance, and support to empower them to take an active role in their healthcare and manage their chronic conditions effectively.
  • Monitoring and Evaluation: Regularly monitor patients' progress, evaluate the effectiveness of care plans, and make adjustments as needed to ensure optimal outcomes.
  • Communication and Collaboration: Effectively communicate with patients, families, healthcare providers, and other stakeholders to ensure seamless care coordination and promote a culture of patient-centered care.
Requirements
  • Education and Experience: Registered nurse with current unrestricted license to practice in the state or a state with licensure reciprocity, and a minimum of 2 years of direct clinical care experience.
  • Skills and Qualifications: Excellent communication and interpersonal skills, basic typing and computer knowledge, and the ability to work effectively in a fast-paced environment.
  • Certifications and Training: Successful completion of disease management orientation program and certification in CCM or CHC is encouraged.
