Job Summary

We are seeking an experienced Advanced Care Manager to join our team in delivering high-quality, patient-centered care to individuals with serious mental illness.

About the Role

The ideal candidate will have a strong background in primary care and value-based care, with experience working with underserved populations. They will be responsible for performing whole-person clinical assessments, identifying and remedying care gaps, and coordinating care among medical and behavioral health providers.

Key Responsibilities
  • Perform comprehensive clinical assessments to identify behavioral and physical health conditions
  • Organize information to support successful disability applications
  • Make individualized recommendations and referrals for programs/care provided by our organization or outside providers
  • Lead the medical component of team-based care, training and advising Guides and Community Resources Guides on clinical issues and treatments
Requirements
  • 3+ years clinical care as a Nurse Practitioner, ideally in a primary care setting or Value-Based Care
  • Masters degree or higher
  • Experience working with and managing underserved populations with challenges such as behavioral health and/or SUD conditions
  • Home visits are a major component of this job
  • Ability to maintain professional boundaries and engagement skills with a population with challenges and in non-traditional work conditions
