Lead Physician for Value-Based Primary Care

As a fully capitated Medicare Advantage provider, we take a holistic approach to patient care, incorporating preventive measures, case management, and social services to ensure our patients receive comprehensive support.
About the RoleIn this key position, you will serve as a quarterback or general manager for total patient care, working closely with a dedicated care team that includes nurses, case managers, and consulting specialists. Your expertise will be essential in delivering exceptional outcomes for our patients, who often face significant barriers to accessing quality healthcare.
The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for working with seniors. You will enjoy a manageable patient panel size of 400 patients, allowing for regular and meaningful interactions. Our practice model prioritizes work-life balance, with a Monday-to-Friday schedule, no nights or weekends, and limited call duties.
