Job Summary

We're hiring a Senior Patient Care Coordinator to lead our team in Fairfield County, CT.

In this role, you'll oversee patient care services, develop care plans, and supervise caregivers. You'll also focus on building strong relationships with clients and caregivers, ensuring exceptional experiences throughout their journey.

This position requires a registered nurse (RN) with a valid CT & NY license, as well as experience in in-home care or private care. A valid driver's license and full Covid-19 vaccination are also necessary.

Our estimated salary range is $90,000 - $105,000 per year, based on your qualifications and experience.
