Job Title: Direct Support Professional - Floater/Responder

Job Summary:

Momentum, a leading provider of support services for individuals with disabilities, is seeking a highly motivated Direct Support Professional - Floater/Responder to join our team. This unique position involves providing support within our residential, adult day, and Refresh programs, as well as responding to emergency crises when needed.

Responsibilities:

  • Provide flexible support in different roles and settings as needed within Momentum's Refresh Program.
  • Act as the first point of contact to address and resolve any urgent situations, including behavioral issues, medical emergencies, or unexpected challenges that may arise.
  • Provide individualized care and support to clients based on their unique needs, assisting with activities of daily living, personal care, and behavioral interventions.
  • Maintain accurate records and documentation of client progress, incidents, and interactions according to established procedures and protocols.
  • Encourage and support clients in participating in community activities, fostering social connections, and promoting inclusion.
  • Communicate effectively with clients, their families, and the broader support team to ensure a holistic approach to care.
  • Foster a positive, inclusive, and empowering environment that promotes clients' independence and self-advocacy.

Requirements:

  • Willingness to travel for up to two weeks.
  • Team player attitude and a strong commitment to helping others.
  • Must be 21 years of age or older.
  • Valid and current proof of legal right to drive in California (required).
  • Reliable means of transportation (required).
  • Clean driving record with DMV driver's record printout (required).
  • High School Diploma or equivalent (required).
  • Experience working with developmentally disabled individuals (highly preferred).
  • Successful completion of all post-offer, pre-employment screening requirements, including background check, drug screening, health screening, TB screening, and proof of eligibility to work in the United States.
  • Ability to perform physical tasks, including running, pushing, pulling, bending, squatting, kneeling, and lifting up to fifty (50) pounds.
