Job Summary

As a RN Care Manager, you will play a critical role in facilitating patient care, enhancing quality of clinical outcomes, and promoting patient satisfaction while managing the cost of care.

Key Responsibilities
  • Assess patient needs and develop effective care plans to ensure timely and safe transitions to community settings.
  • Collaborate with interdisciplinary teams to eliminate barriers to efficient care delivery and reimbursement.
  • Apply process improvement methodologies to evaluate outcomes of care and identify areas for improvement.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Participate in departmental meetings and share knowledge related to the practice of case management.
Requirements
  • Bachelor of Science degree in nursing preferred.
  • Associates in Nursing required.
  • Minimum of one (1) to three (3) years of hospital or related experience is required.
  • Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's).
  • Bilingual language skills preferred (Spanish).
  • Basic computer skills.
  • Current California Nursing license.
  • Current Basic Life Support (BLS).
  • Certification in Case Management is preferred.
