Job Opportunity

Mobile Healthcare Partners Inc is seeking a skilled Physician Assistant to join its growing team in Telemedicine.

Key Responsibilities:

  • Lead the Acute Management Team for homebound patients that Mobile Primary Care is the Primary Care Provider.
  • Coordinate daily with triage staff to identify and complete acute telemedicine visits and subsequent service coordination.
  • Lead the ER/Hospital Avoidance Team.
  • Determine when to send a Mobile Primary Care Medical Assistant or Nurse to the patient home with telemedicine and acute care stabilization materials.
  • Coordinate direct Sub-Acute Rehab stays.
  • Visit patients in their homes to stabilize and coordinate care.
  • Maintain accurate and up-to-date electronic health records for each patient.
  • Review reports on patient outcomes and contribute to quality improvement initiatives.

Requirements:

  • A minimum of 2 years clinical experience as an NP/PA in Family Medicine or Internal Medicine Environment; 3-5 years is preferred.
  • NP/PA must have a current and unrestricted DEA License.
  • Current and unrestricted New York state license.
  • Possess Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification.
  • Working knowledge of Microsoft Office Software.
  • Exceptional diplomatic communication skills and experience in handling difficult customer situations.
  • Must be detailed oriented and have the ability to multi-task.
  • Master's Degree in Nursing with completion of a university-based program for Nurse Practitioner.
  • Relevant certification from American Nurses Association or another nationally recognized certifying body.
