About the OpportunityThis outpatient-only position offers a unique chance to focus on building relationships with patients, promoting wellness, and making a meaningful difference in the lives of seniors. With an established patient panel size of 600 patients on Medicare and Medicare Advantage, you will have the opportunity to develop long-term relationships with your patients and provide comprehensive care.

- Flexible Encounters: Offer in-person, virtual, or telephonic encounters, including annual comprehensive visits, transitional care, and acute/urgent visits.
- Exceptional Work-Life Balance: Enjoy a 30-minute to 1-hour appointment slot for all patients, with adequate time without rush.
- APP Support: Manage a patient panel size of 600 or 1,000 with APP support.
