Northern Light Health is seeking a Registered Nurse II to join our Home Health team. As a Registered Nurse II, you will be responsible for providing high-quality patient care to patients in their homes. This is a full-time position located in South Portland, Maine.
Responsibilities:
- Provide comprehensive and holistic assessments to patients in their homes.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Provide education and support to patients and their families to promote health and wellness.
- Document patient care and progress in a timely and accurate manner.
- Participate in performance improvement initiatives to enhance patient care and outcomes.
Requirements:
- Current licensure as a Registered Nurse in the State of Maine.
- 1 year of clinical experience as a Registered Nurse.
- Associate's degree in Nursing or Bachelor's degree in Nursing preferred.
- Valid driver's license with reliable transportation and agency required auto liability insurance.
Working Conditions:
- Lifting, moving and loading 30 to 50 pounds.
- Alternate shift schedules (day, evening, nights, weekends).
- Prolonged periods of sitting, standing, and walking.
- Potential exposure to hazardous materials, noxious odors, and diseases or infections.
