Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our Hospice team. As a Home Health Aide, you will provide essential care and support to patients in their homes, working under the guidance of a Registered Nurse or Therapist.

Key Responsibilities
  • Assist patients with daily living activities, such as bathing, dressing, and toileting, under the supervision of a Case Manager.
  • Review care plans with patients and notify Case Managers of any issues or concerns.
  • Provide emotional support and involve patients in their care, explaining the care being provided.
  • Complete mobile care tasks upon arrival and departure from patients' homes, including mileage logs.
  • Work cooperatively with patients, care providers, and family members to ensure seamless care.
Requirements

To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. You will also need to be able to maintain patient confidentiality and adhere to Agency policies and procedures.
