Join Our Hospice Team as a Home Health Aide

We are seeking a compassionate and dedicated Home Health Aide to join our interdisciplinary Hospice team. As a Home Health Aide, you will provide patients with care in the comfort of their own home and/or similar setting, serving patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties.

Key Responsibilities:
  • Provide direct assistance with activities of personal care, maintenance of patient health, and Agency facilitation of treatment for patients.
  • Work cooperatively with patients, care providers, and family members, as well as with appropriate Agency staff.
  • Regularly complete home maintenance per plan of care, including house cleaning, washing dishes, laundry, meal preparation, and transportation.
