About the Role

We are seeking a compassionate and skilled Home Health Aide to join our Hospice team. As a Home Health Aide, you will provide high-quality care to patients in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment under the direction of a Registered Nurse or Therapist.
  • Support patient care by reviewing care plans, notifying Case Managers of issues or concerns, and completing mobile care upon arrival and departure from patients' homes.
  • Work cooperatively with patients, care providers, and family members to ensure seamless care delivery.
  • Regularly complete home maintenance tasks, such as house cleaning, laundry, and meal preparation, as outlined in the patient's plan of care.
