Key Responsibilities:
- Assessment and Evaluation: Conduct thorough assessments and evaluations of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
- Therapy Planning: Develop and implement individualized therapy plans in accordance with the plan of care, using evidence-based techniques and in coordination with other members of the healthcare team.
- Patient Care: Provide occupational therapy services to patients in their homes, promoting independence, mobility, and overall well-being.
- Communication: Maintain effective communication with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements:
- Licensure: Current Occupational Therapy licensure in the state of practice.
- Certification: Current CPR certification.
