Job Title: Mental Health Outreach and Navigation Team Liaison

Job Summary:

We are seeking a highly skilled Mental Health Outreach and Navigation Team Liaison to join our team at Coordinated Care Services, Inc. (CCSI). As a key member of our outreach team, you will play a vital role in connecting high-need individuals with essential services and support.

Key Responsibilities:

  • Serve as the first point of contact for high-need individuals in the community, providing support and guidance to help them access necessary services.
  • Collaborate with community partners, including healthcare and human services, social services, and law enforcement, to ensure a comprehensive and coordinated approach to support.
  • Process incoming referrals, track progress, and maintain accurate documentation and client records.
  • Develop and implement system-wide treatment and support plans, incorporating professional and natural support.
  • Partner with individuals to assess their needs, explore supports, and identify solutions, leading to connections with individualized and self-defined recovery services.
  • Conduct timely engagement activities in various settings, building positive relationships with adult serving systems.

Requirements:

  • Bachelor's degree in Human Services or related field, with 2 years of relevant professional experience, or equivalent education and work experience in mental health, substance use, developmental disability, and/or criminal justice systems.
  • Satisfactory completion of all required background screenings.
  • Full vaccination against COVID-19, with reasonable accommodations made for qualified religious or medical/disability needs.
  • Knowledge of community resources and providers, familiarity with mental health, medical, substance use, and criminal justice terminology, and excellent written and verbal communication skills.
