Job Title: Case Manager II

Catholic Charities is seeking a dedicated and compassionate Case Manager II to join our team at the Higher Ground - St Paul location. As a Case Manager II, you will work directly with assigned clients to develop and implement personalized service plans, ensuring their needs are met and they have access to necessary resources.

Key Responsibilities:

  • Conduct comprehensive assessments to identify client needs and develop tailored service plans.
  • Collaborate with community partners and Catholic Charities services to provide clients with comprehensive support.
  • Advocate for clients' rights and interests, ensuring they receive fair treatment and access to benefits.
  • Maintain accurate and up-to-date case documentation, adhering to Catholic Charities' and regulatory requirements.
  • Participate in education and outreach efforts to promote Catholic Charities' services and programs.
  • Work closely with the supervisor and team members to ensure seamless client care and coordination.

Requirements:

  • Bachelor's degree in human services or a related field (or equivalent experience).
  • Minimum of two years of experience working with a specific client group (or equivalent experience).
  • Valid Minnesota driver's license and proof of current insurance coverage (if transporting clients).
  • Ability to work with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness.

Physical Requirements:

  • CPR certification.
  • Ability to lift up to 25 pounds without assistance.
  • Ability to lift up to 50 pounds without assistance.
  • Ability to physically restrain clients (if necessary).
