Community Care Partners is seeking a skilled Licensed Practical Nurse to join our team. As a key member of our healthcare team, the LPN will provide high-quality patient care services, working closely with providers and other medical staff to ensure seamless care delivery.
Key Responsibilities- Patient Care Services: Prepare patients for examination, record vital signs, and provide treatment as applicable.
- Medical Support: Assist in determining medical problems, initiate and maintain intravenous lines, administer fluids and medications as directed by provider orders.
- Collaboration and Communication: Collaborate with providers and other medical staff when examining or treating patients, giving injections, removing sutures, and applying splints and dressings.
- Medication Management: Authorize medication refills as per provider direction and provide prescription information to pharmacies.
- Lab and Testing: Collect blood, tissue, and other laboratory specimens through routine in-house testing procedures, and analyze and prepare for testing, log results, and perform lab call-backs.
- Exam and Treatment Rooms: Prepare and maintain exam/treatment rooms, select, setup, sterilize, and maintain medical supplies and equipment for procedures.
- Documentation and Scheduling: Document basic patient care and procedures, utilizing appropriate forms and electronic medical records, and schedule patients for tests and follow-up appointments.
- Patient Education: Instruct patients and family regarding treatment and medication instructions.
- Team Collaboration: Collaborate, take direction, and initiate actions that will allow the assumption of cross-functional team actions to ensure seamless patient care.
- Education: High school graduate or equivalent is required.
- Certificate/License: Current LPN license. Current BLS certification is required.
- Experience: A minimum of one - three years of relevant experience is preferred.
